Procedures for Consultation

• If you have left your registration card at home or lost it, please report to the counter for revisiting patients. For people who wish to apply for services in relation to medical welfare (including health care related to disabilities and rehabilitation) and industrial accidents, please submit applications to the office.
• Referrals and radiographs issued or provided by other hospitals or clinics shall be submitted to the counter for first consultation.
• On the first consultation day of each month, your health insurance care and other certificates issued for recipients of services shall be confirmed.
• External and internal prescriptions shall be submitted to the “external prescription consultation counter” and “drug counter”, respectively, to receive drugs.
• People who have made an appointment for hospitalization shall receive “a hospitalization appointment card” and “handbook for hospitalization”.
